Our license, the posted patient rights, and the current fee schedule are displayed in the reception area. Ask any staff member if you would like a copy of anything you see.
Referring someone to us
We accept referrals from courts, probation, health plans, physicians, schools, other providers, and from people referring themselves.
Call 520-213-7435 to discuss a referral, or send it to the address above with the person's contact details and any assessment already completed. We will follow up on it.
Insurance and payment
Our current fee schedule is posted at the office and available on request. Call to discuss coverage, payment, and what your care will cost before you commit to anything.
Already a patient?
Call 520-213-7435 during opening hours to reschedule, request records, or raise a concern. If you are in crisis outside our hours, use the numbers at the bottom of this page.
